The Department of Agriculture, through its Bids and Awards Committee (BAC), intends to lease a venue including accommodation for the Office of the Undersecretary Year-end Assessment and Strategic Planning for FY 2019 on November 26 – 29, 2018 at Region IV – A.

 

The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.

 

As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly __________ for the item described below

subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:

 

  1. Business Permit;
  2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
  3. PhilGEPS Certificate/Supplier Organization Profile/Registration.
