The Department of Agriculture, through its Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the activity of the National Convergence Initiative, “INTEGRATED ECOSYSTEMS MANAGEMENT – CONVERGENCE AREA DEVELOPMENT PLAN (CADP) WORKSHOP FOR THE IMPLEMENTERS IN THE MUNICIPALITIES OF ALICIA, CANDIJAY GUINDULMAN, MABINI, PILAR AND UBAY” to be conducted on January 23-27, 2017 in Bohol Province (Region7) for 70 participants.
The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.
As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than December 14, 2016, at exactly 12:00 n.n. for the item described below, subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ).
*Inclusive of FOOD for the duration of the activity, Breakfast, AM Snack, Lunch, PM Snack, Dinner. For ACCOMMODATION, Triple Sharing. For VENUE, Conference Hall with 70 pax to 100 pax capacity
Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:
1. Business Permit;
2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
3. PhilGEPS Certificate of Registration.
