The Department of Agriculture, through its Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the conduct of the 2nd Semester Consultative Meeting Cum Workshop for Regional Information Officers, Library and AFID staff on September 12 – 15, 2018 in Region III.

The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.

As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than ____ 2018, at exactly 5:00 p.m. for the item described below,

subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:

1. Business Permit;
2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
3. PhilGEPS Certificate of Registration.
