The Department of Agriculture-Bureau of Agriculture and Fisheries Standards (DA-BAFS), through the DA – Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the Seminar Awareness on Food Safety Related Philippine National Standards to be conducted on 20-21 November 2017 in Region X (Cagayan De Oro).

 

The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.

 

As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than _________, at exactly ________ for the item described below:

*** Live-in participants (Accommodation with full board meals) for 2days (November 20-21, 2017) (10 pax X P1,800.00 X 2 days)

***Live out participants (AM Snacks, Lunch, PM Snacks) for 1 day (November 21, 2017) (35 pax X P1,000.00 X 1 day)

subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:

 

  1. Business Permit;
  2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
  3. PhilGEPS Certificate of Registration.
