The Department of Agriculture, through its Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the activity of the National Convergence Initiative, “Convergence Area Development Plan Training Workshop for Mt. Haguimitan covering the Municipalities of Mati, San Isidro, and Governor Generoso, Davao Oriental from October 23-27, 2017 in Davao Oriental (Region XI) for 70 pax.
The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.
As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly ___________ for the item described below:

Inclusive of FOOD, ACCOMMODATION and VENUE for the duration of the NCI Workshop.
FOOD: Breakfast, AM Snack, Lunch, PM Snack and Dinner
ACCOMMODATION: Can accommodated the participants
VENUE: Conference Hall with 70 to 100 participants
(70 pax X P1,300.00 X 5 days)
subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:
- Business Permit;
- Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
- PhilGEPS Certificate of Registration.
