The Department of Agriculture, through its Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the activity of the National Convergence Initiative, “Convergence Area Development Plan (CADP) Workshop for Suawan-Kulafu Subwatershed in Marilog District,” to be conducted on October 9-13, 2017 in Davao City (Region XI) for 70 Participants.

The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.

As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly ___________ for the item described below:

1. Business Permit;
2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
3. PhilGEPS Certificate of Registration.
