The Department of Agriculture, through its Bids and Awards Committee (BAC), intended to lease a venue including accommodations and meals, for the activity of the National Convergence Initiative, “Convergence Area Development Plan (CADP) Workshop using the Integrated Ecosystem Approach (IEM) for the Tayak Adventure Nature Farm and Wildlife (TANAW) Convergence Area in Laguna” for 65 pax to be conducted on August 21-25, 2017 in Region IV-A.

The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.

As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly 12:00 n.n. for the item described below:

***Inclusive of: (1) Food for the duration of the Activity, Breakfast, AM Snack, Lunch, PM Snack, Dinner.

(2) Accommodation can accommodate for 65 participants,

(3) Venue – Conference Hall with 65 pax

subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:

1. Business Permit;
2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
3. PhilGEPS Certificate of Registration.
