The Department of Agriculture, through its Bids and Awards Committee (BAC), intends to lease a venue including accommodations and meals, for the Public Consultation for the Development of Explanatory Brochures on PNS/BAFPS 09:2004 Fresh Fruit Pineapples – Specification on May 21-23, 2018 in Region XII (General Santos)
The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.
As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly __________ for the item described below

***Lease of Venue Including Food and Accommodation
A. Live-in participants (accommodation and full board meals) for 3 days (May 21-23, 2018). Inclusive of use of function/conference room with projector, free wi-fi, audio visual system and free flowing coffee. 6 pax X PhP1,800.00 X 3 days = PhP32,400.00
B. Live-out participants (AM Snacks, Lunch and PM Snacks) for 1 day (May 22, 2018) 50 pax X PhP1,000.00 X 1 day = PhP50,000.00 |

- Business Permit;
- Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
- PhilGEPS Certificate/Supplier Organization Profile/Registration
