The Department of Agriculture, through its Bids and Awards Committee (BAC), intends to lease a venue including accommodations and meals, for the Seminar-Workshop 1: Strategies for the Development of National Strategic Plan; Information Sharing/Capability Building; Plan of Action for Obtaining ISO 17025 Accreditation for DA Laboratories to be conducted on 22-24 May 2018 in NCR.
The lease of venue and accommodations will be undertaken in accordance with Section 53.10 of Revised Implementing Rules and Regulations of Republic Act No. 9184.
As such, you are invited to submit your establishment’s quotations/proposals duly signed by you or your duly authorized representative not later than __________, at exactly __________ for the item described below

B. Live-out participants (AM Snacks, Lunch and PM Snacks) for 3 day (May 22-24, 2018) 15 pax X PhP1,000.00 X 3 day = PhP45,000.00 |
subject to the Terms and Conditions provided at the dorsal portion of the Request for Quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal:
- Business Permit;
- Electronically Filed Income or Business Tax Return [Filed through the BIR Electronica Filing Payment System (eFPS)]; or, Annual Income or Business Tax Return; and
- PhilGEPS Certificate/Supplier Organization Profile/Registration
